Оказывается, mysql odbc connector пытается подключиться через сокет, если имя хоста совпадает с localhost. Достаточно использовать 127.0.0.1 вместо localhost.

Столкнулся с такой же проблемой (как в первом посте).
Пишу для тех, кто столкнётся с ней и найдёт эту тему в поисковиках.
Скорее всего проблема в mysql odbc connector.
Убедиться, что проблема именно в нём, достаточно просто. Прежде всего, в логах не будет ничего похожего на [unixODBC][Driver Manager], как в первом посте. Также в логах mysql сервера не будет отмечено никаких попыток подключения, так как подключение, собственно, и не происходит.
После этого нужно настроить DSN запись для базы mysql используя драйвер, настроенный согласно руководству ERA и с помощью утилиты isql попробовать к ней подключиться. Подключиться не получится, тк произойдёт floating point exception.
Если все проверки заканчиваются именно так, то проблема именно в mysql odbc connector.
Чтобы решить эту проблему нужно скачать с сайта mysql последнюю версию этого коннектора, распаковать где-нибудь и настроить её использование.

Скачал виртуалку http://download.eset.com/download/ra/v6/Appliances/era_server.ova
Развернул. Пользуюсь.
Это оказалось проще, чем искать в чём проблема.

Сапорт ответил:

1) Установите, пожалуйста, требуемые библиотеки:apt-get install unixodbc libmyodbc

2) Создайте файл /etc/odbcinst.ini и заполните его следующим содержанием:

[MySQL]
Description = ODBC for MySQL
Driver = libmyodbc.so
Setup = libodbcmyS.so
Driver64 = libmyodbc.so
Setup64 = libodbcmyS.so

Путь до библиотек libodbcmyS.so и libmyodbc.so должен быть указан полность.
Директорию их устаноки можете обнаружить с помощью поиска:
find / -name 'lib*odbc*.so'

3) В скрипте установки в качестве значения для параметра --db-driver следует
указывать инстанс, который указан в квадратных скобках в файле odbcinst.ini,
в нашем случае: MySQL

Попробую поставить на ubuntu 14.04 как в предложенном решении.
